Terms and Conditions: In consideration of the rate charged, it is agreed that the value of this statement, weather consisting of one or more packages, boxes, etc. is not greater than $1,000.00 and that the liability of MERCURY for loss or damage in connection with such shipment is limited to $1,000.00 unless a greater value is declared at the time order is placed. For additional coverage (over $1,000.00) the rate is $1.00 per $1,000.00 of coverage up to $50,000.00. Shipper must provide own insurance for value in excess of $50.000.00. MERCURY shall not be liable for loss or damage to shipment improperly packed or labeled or for loss or damage caused by delay. All complaints regarding loss or damage of any kind must be submitted to MERCURY in writing seven (7) days of receipt of the shipment. Claims not made within a seven day period shall be deemed waived by all parties involved with shipment.
